Connect2 is the technology platform co-designed by HealthierHere with community partners to facilitate sharing information among participants in a Community Information Exchange. Connect2 comprises Connect2 Exchange, the “back-end” system which stores and shares client information among participants in the network, and Connect2 Coordinator, a web-based client management application built on Connect2 Exchange that allows participants in the network to manage information about clients, coordinate care for clients, and make referrals to the services clients need.

A Community Information Exchange (CIE) is a collaborative network of organizations (social services, hospitals, physical and behavioral health centers and providers, etc.) serving a community that shares information (data) to improve care coordination and better connect communities to care. In a CIE, technology is used as a tool to support the exchange of information.

Breaking down silos between information pathways via integration with the Connect2 Exchange will have many benefits for community members. It provides a consensus view of client information across network participants, a comprehensive list of care team members, a directory of resources available to meet client needs, a history of client referrals that may be useful in meeting a new or recurring need for services, and a common hub for making referrals to and among network participants. Access to this shared data resource will create a more client-centered experience by improving cross care team collaboration and enabling care teams’ access to the most up to date client, care team, and referral information.

In addition to receiving the benefits of Connect2 Exchange, adopting Connect2 Coordinator will allow organizations to participate in the CIE through the use of an intuitive client management platform that allows users to manage client intake and client information, assess client needs, search for and refer clients to services to meet their needs, manage and streamline case worker workload and workflow, report on case progress and client services, track billable services, and securely and privately communicate with other Connect2 Coordinator users.

Connect2 Exchange and Connect2 Coordinator implement security safeguards that help users of the platforms to be compliant with the security requirements in the HIPAA Security Rule, and the Connect2 Privacy Framework was co-designed with community and in consultation with HIPAA privacy experts to include privacy protections to help users of the platform to be compliant with the privacy requirements in the HIPAA Privacy Rule.

Yes. The security of the Connect2 technology hosting environment is assured through a collection of third-party security assessments and certifications, including FedRAMP, FIPS, and HITRUST CSF certifications. The processes of our technology vendor who develops the technology are likewise certified to incorporate robust security practices audited by an independent third party for HIPAA compliance.

Connect2’s Privacy Framework was co-designed by HealthierHere with community partners to ensure all network participants work together to protect client privacy. The Framework comprises five elements: our Privacy Policy that describes privacy protections and client rights regarding their data, our Client Authorization by which a client may give permission to collect, use, and share their data in the provision of services, our Participant Agreement which establishes obligations among network participants when using Connect2, our Data Sharing Agreement which establishes how network participants may use and will protect client data, and our model Business Associate Agreement which adds provisions to our Data Sharing Agreement needed by participants subject to HIPAA.

Organizations, including Accountable Communities of Health, who integrate their existing systems with Connect2 Exchange can gain access to any or all of the Exchange functionality. Those who adopt Connect2 Coordinator will have access to both the Connect2 Exchange functionality and the Connect2 Coordinator functionality.
